In order to be eligible for either loan repayment program, applicants must meet the eligibility requirements (i.e. citizenship, health professional license, educational loan indebtedness, employment at an approved facility).
Individuals can apply to both the NHSC and CT Student Loan Repayment Programs at the same time. If approved, it is the responsibility of the applicant to choose one program, and recuse themselves for consideration in the other.
Only completed applications will be considered for the SLRP. The award selection process will be based on multiple factors, including geographic and discipline-specific gaps in Connecticut.
Not all eligible applicants who submit completed applications are guaranteed an award.

Proof of Status (residency) as a U.S. Citizen or U.S. National
Document: A copy of a birth certificate OR ID page of a current, unexpired U.S. passport.
Current CV/Resume
Document: No more than 2 pages, including Titles, License/s, Employment and Education.
Loan Information Verification
Document: Applicants must provide two types of documentation for each loan that is being submitted for consideration: (1) an account statement and (2) a disbursement report. See NHSC/LRP guidance, page 10 NHSC Loan Repayment.
