How To Apply

Interested applicants for the CT SLRP should familiarize themselves with the NHSC Loan Repayment Program requirements before consulting the SLRP application checklist. 


In order to be eligible for either loan repayment program, applicants must meet the eligibility requirements (i.e. citizenship, health professional license, educational loan indebtedness, employment at an approved facility).

Individuals can apply to both the NHSC and CT Student Loan Repayment Programs at the same time.
 If approved, it is the responsibility of the applicant to choose one program, and recuse themselves for consideration in the other. Only completed applications will be considered for the SLRP. The award selection process will be based on multiple factors, including geographic and discipline-specific gaps in Connecticut.


Not all eligible applicants who submit completed applications are guaranteed an award.

Disclaimer: The Connecticut Student Loan Repayment Program (CT SLRP) is modeled on the National Health Service Corps Loan Repayment program (NHSC/LRP).  For the purposes of this document, all reference to the NHSC/LRP can be found in the FY24 NHSC Loan Repayment website.


To learn more about different types of loan repayment available through the National Health Service Corps, please consult Apply for Loan Repayment | Bureau of Health Workforce (hrsa.gov). Student Loan Repayment (NHSC/LRP and CT SLRP) only applies to educational loans for the applicant.


  • Step 1

    Consult NHSC Loan Repayment Program for confirmation of:

    • Eligible Disciplines

    • Hours of Direct Patient Care


    Please note:  Full-time is defined as 36 hours per week, part-time 18 hours per week.

  • Step 2

    Log in to HRSA Workforce Connector to verify the HPSA score and eligibility of the current facility where you are employed.

  • Step 3

    Review ‘Frequently Asked Questions’ on the CT SLRP website.


    If you have a question that does not appear, please reach out to the CT SLRP team via email or phone.

  • Step 4

    Make sure to review the CT SLRP application deadline and where to submit the application and supporting documents.


    Applicants should prepare their supporting documents before starting the application.

  • Step 5

    Assemble the following documents to upload to the CT SLRP application.

    You are encouraged to gather your information before beginning the process, as some documents may require additional time to obtain from lending or training institutions. 


    Save each document as a PDF file before uploading it to your application and name the document to reflect applicant’s First Initial_Last Name_CT SLRP_Type of Document (ApplicationConsent,NPDBSelfQuery, Medical License,  Citizenship, Loan Information, CV).

    Example:  J. Smith_CT SLRP_ApplicationConsent.


    Do not attempt to upload files larger than 5MB, or files saved as TIFF, JPEG, PNG, HEIC, or TXT.


    Required Supporting Documentation:

    • National Practitioner Data Bank Self-Query
    • Health Professions License

    Documents: 

    Applicants will be required to submit proof of their current, full, permanent, unencumbered, unrestricted health professional license, certificate, or registration (whichever is applicable) with an expiration date. 


    Proof of Status (residency) as a U.S. Citizen or U.S. National 

    Document:  A copy of a birth certificate OR ID page of a current, unexpired U.S. passport.


    Current CV/Resume 

    Document: No more than 2 pages, including Titles, License/s, Employment and Education.


    Loan Information Verification

    Document: Applicants must provide two types of documentation for each loan that is being submitted for consideration: (1) an account statement and (2) a disbursement report.  See NHSC/LRP guidance, page 10 NHSC Loan Repayment.
